Observable language matters
Replace labels (“defiant”) with observable behavior (“left seat without permission during independent work”) so the team can coach strategies instead of debating character. Note antecedents — transitions, peers, hunger, unstructured time — patterns show up faster on paper than in memory.
Intervention should name what you tried in the moment (proximity, choice, break, check-in). Parent / admin contact records phone attempts, emails sent, or referrals without duplicating confidential files — follow your district’s policy for official discipline records.
