Your vet's schedule is the truth — this page is the place you write it down
Dog vaccine schedules depend on age, lifestyle (city dog vs. trail dog vs. boarder), region (Lyme is endemic in the Northeast US, Lepto in much of the rural South), and breed (some giants get adjusted booster intervals). The AAHA core / non-core distinction matters: Rabies is legally required in most US states and Canadian provinces, while Lyme and Leptospirosis are *recommended where exposure exists*.
This sheet preprints a typical AAHA puppy series — DHPP at 6–8 / 10–12 / 12–16 weeks, with Rabies at 14–16 weeks — plus the annual + 3-year adult cycle. Every row has a Date given, Booster due, and Vet / lot # column so the page works as a portable proof-of-vaccination card for boarding, day-care, and travel.
Monthly heartworm and flea / tick preventatives are on the bottom rows because they're the part owners forget — and unlike vaccines, missing a month *can* mean infection. Set a phone alarm.
Pair with the Pet Medication Schedule for daily dosing and the Pet First Aid Kit Checklist for emergencies. *This template summarizes typical guidelines — your veterinarian's recommendations override anything printed here.*
